Completing and Submitting Volunteer Waivers:

• Download the Adult Volunteer Waiver and Appearance Waiver
• If minors are participating in the Day of Caring project, please have each minor complete the Minor Volunteer Waiver Form
• Have each volunteer complete the appropriate forms and return to your corporate team leader
• All volunteer waivers must be submitted to Pittsburgh Cares (239 Fourth Ave., Suite 1007 Pittsburgh, PA 15222) via mail within one week of the project
  

Completing and Submitting the T-Shirt Request Form:

• T-Shirts for Days of Caring can be picked up at the Pittsburgh Cares office (239 Fourth Ave., Suite 1007 Pittsburgh, PA 15222) at a time coordinated between a company and Pittsburgh Cares daysofcaring@pittsburghcares.org.
• Download the T-Shirt Request Form
• Complete the form
• Fax (412-456-2212), email, or mail (239 Fourth Ave., Suite 1007 Pittsburgh, PA 15222) the T-Shirt Request form to Pittsburgh Cares
