BUSINESS CARES: DAYS OF CARING: ADDITIONAL FORMS

Completing and Submitting an Agency Waiver Form:

• All waivers must be completed and submitted prior to a Day of Caring project
• Download the Agency Waiver Form
• Review and sign the waiver
• Fax (412-456-2212) or mail (535 Smithfield Street, Ste. 744 Pittsburgh PA 15222) the waiver to Pittsburgh Cares

Submitting a Paint Request Form:

• Paint can be requested for projects taking place between fall 2008 and spring 2009.
• Paint Request Forms must be submitted by August 31, 2008 and AT LEAST TWO WEEKS prior to your project to allow for processing and delivery. There will be no exceptions.
• All paint shipments will be shipped between August 1 and September 30.
• Paint Requests must be made using the PPG Paint Donation Online Request Form. This form can be reached clicking HERE.
• Please direct all Paint Request Form questions to Daniel Horgan at Pittsburgh Cares via email dhorgan@pittsburghcares.org or phone (412) 471-2114. To answer technical questions, Daniel will put you in direct contact with the appropriate person at PPG.
