BUSINESS CARES: DAYS OF CARING: ADDITIONAL FORMS

Completing and Submitting an Agency Waiver Form:

• All waivers must be completed and submitted prior to a Day of Caring project
• Download the Agency Waiver Form
• Review and sign the waiver
• Fax (412-456-2212) or mail (239 Fourth Ave., Suite 1007 Pittsburgh, PA 15222) the waiver to Pittsburgh Cares

Submitting a Paint Request Form:

• Paint can be requested for projects taking place between fall 2009 and spring 2010.
• Paint Request Forms must be submitted by July 31, 2009. There will be no exceptions.
• All paint shipments will be shipped between August and September 2009.
• Please direct all Paint Request Form questions to Pittsburgh Cares via email at daysofcaring@pittsburghcares.org or phone (412) 471-2114. To answer technical questions, Pittsburgh Cares staff will put you in direct contact with the appropriate person at PPG.
